How it works
For anyone with unexplained symptoms, new autoimmune or inflammatory lab results or an established rheumatic or autoimmune condition.
Includes:
- 60 minute in-person visit with Dr. Ortiz
- Initial assessment and orders for necessary testing
- 20 minute virtual visit for test result review & recommended next steps
After your initial consultation, you can choose to continue your care with Dr. Ortiz and Paradigm Health or work with another doctor.
If you decide that Paradigm Health is where you want to be (good choice!), depending on if you have a new diagnosis or just want to continue your care with Paradigm – we have a plan that will work for you.
Each plan includes:
Annual Membership to Paradigm Family Health and all included benefits (including timely visits and direct access to your doctor – great for flares!)
4-6 Rheumatology visits per year, depending on your situation
Rheumatology visits that last as long as you need them to

But I already have a Primary Care Provider…
…do I have to change PCPs in order to see Dr. Ortiz?
You can absolutely take advantage of the Rheumatology Consult with Dr. Ortiz without making any changes to your primary care provider.
However, to become an ongoing patient with Dr. Ortiz, a Paradigm Health membership is required.

Why should I get my rheumatology care with Paradigm Family Health?
Our healthcare system is hopelessly fragmented. Every specialist works in their own silos, never speaking to each other, instead communicating only through faxed medical records that have a lot of words but say very little.
We all suffer when our doctors don’t communicate but it is especially true when you have an autoimmune disease. Those with rheumatic or autoimmune disease need a TEAM to truly feel their best and that team starts with Primary Care AND Rheumatology. Each brings their own expertise and YOU benefit.
Can I start with Dr. Ortiz and then transfer my care to solely the primary care providers at Paradigm Family Health?
It depends. There are some conditions that only require a rheumatologist for a period of time and can then be safely transferred to primary care. These will be handled on a case by case basis with appropriate prorated refunds available. However the majority of autoimmune and rheumatic conditions are lifelong and need both rheumatology and primary care working together.
Does Dr. Ortiz or Paradigm Family Health take insurance?
No. But you can use your insurance for referrals, labs, vaccines and imaging.
What if I need labs or imaging done?
As a Paradigm Member, you will be benefit from negotiated wholesale prices with local labs. Price lists are available in the clinic – no hidden fees or big bills a few weeks later!! However, most specialized autoimmune lab tests and imaging are not included in wholesale pricing and can be covered by your insurance.
Can I reach the doctor after hours?
Of course!!! This clinic was created so you can call your doctor when you need them and is the benefit of being a Paradigm Health member. When you choose to get your care with Dr. Ortiz and Paradigm Health you have access to your Paradigm Health Providers when you need them and timely visits with Dr. Ortiz. No more unanswered portal messages or waiting months for appointments.
Why do I need to commit to a year membership?
Autoimmune and rheumatic diseases unfortunately do not have a cure. Dr. Ortiz has over 15 years experience treating all kinds of autoimmune and rheumatic conditions and understands the time and attention needed to get these diseases under control. Our year memberships are designed to ensure you have the access to your specialist and primary care team you need to feel your best.
What if I move or have to leave the membership before the end of my annual term?
We will gladly refund the prorated amount for your visits and can securely send your medical records to your next provider. However, please note that we will not be able to continue managing your rheumatological or autoimmune disease.
What if I need more care or more visits than what is prescribed in my subscription plan?
Our goal is to cover all your autoimmune needs, ensuring you have appropriate access to both rheumatology and primary care. If you find yourself needing more access, we will not leave you hanging! We will address each case and their individual circumstances – but rest assured, once you are a Paradigm member, you will get the care you need.
What is Direct Primary Care?
The American Academy of Family Physicians defines DPC as an innovative model where patients typically are charged a monthly, quarterly, or annual fee (i.e., a retainer) that covers all or most primary care services. This benefits patients by providing substantial savings and a greater degree of access to, and time with, their physician. Every DPC practice can inherently be different in their structure including pricing and services offered.
I have Medicare, can I see Dr. Ortiz at Paradigm Family Health?
Yes, but your rheumatology and primary care services will not be billed to Medicare. As such you will pay a membership fee just like all of our members. As a Medicare patient, you can use your Medicare or supplement plan for labs, imaging, medications or referrals.
What if I need to be admitted to the hospital?
Paradigm does not have admitting privileges at any hospital. If you need to be hospitalized, we will work to help coordinate care during and after your hospitalization. Remember, you will still need insurance to cover any hospitalization.
